This is an announcement that Tina Christie, will be leaving our clinic and moving to another clinic as of May 30, 2007.

At Inspired Life Health Centre, we strive to Inspire others to grow in positive directions in both their personal and business lives.

If you have had a Naturopathic appointment with Tina at our clinic, we would like to provide you with additional information on what your options are regarding your personal health information and health care options. At your earliest convenience, please contact our clinic at 416-461-8688 or stop by our office to get more information.

How This Change Affects You As A Client of Inspired Life Health Centre

The following section outlines how this change affects you and your personal health information.

1. Your personal health information records (originals) will remain at Inspired Life Health Centre for a minimum period of 10 years. You have the right to access your personal health information by scheduling a meeting with the Health Information Custodian or an authorized agent of Inspired Life Health Centre. Please contact our clinic for more information.
2. You have the following options regarding your continue health care:
   • You may transfer your care to another practitioner at Inspired Life Health Centre.
     If you choose this option, you need only to notify our office upon your next booking. No additional forms are required. Further information on this option can be requested by contacting our clinic.
   • You may transfer your care outside of Inspired Life Health Centre.
     You may continue your care with Tina Christie at her new clinic or you may transfer your care to another third party practitioner or clinic. If you choose either of these options, you will be required to sign a Release of Records Authorization form in-person at our clinic. By signing the Release of Records Authorization form, you are authorizing Inspired Life Health Centre to make one copy of your peronal health information records for your personal use. This will be the copy that you provide to Tina Christie or your new health care practitioner outside of Inspired Life Health Centre. Please note that your original forms will remain at Inspired Life Health Centre.
